Arkansas Traumatic Brain Injury Provider Resource Assessment

The Brain Injury Association of Arkansas is working with the Arkansas Department of Health TBI Project to develop a Resource Directory.  If your organization provides services to individuals or families who have experienced traumatic brain injury (TBI), you are encouraged to complete the following assessment tool.

To be included in the first published version, scheduled for release on November 7, 2008, you must submit the information no later that August 15, 2008.  All information, including accreditations, will be verified.

Be forewarned, the assessment tool is long!  You are encouraged to print off a hard copy (7 pages long).  Gather the needed information and either mail the assessment tool to the address provided on the assessment tool or enter the information online (preferred).

If you enter the information online, the assessment tool will be broken into 5 components so you don't have to enter everything all at once.
